Agreement for Virtual Sessions
COCREATE CHANGE, PLLC
Remote life coaching sessions are conducted by video chat services or by phone and require prepayment. All clients must be age 13 and older. Ages 13-17 require parent/guardian consent.
SCHEDULING/RESCHEDULING FEES
Once booked, appointment changes must be made at least 18 hours in advance of your scheduled appointment to avoid a full client session charge. Your session fee is not refundable due to any reason, including personal emergency, however the session can be rescheduled with 18-hour notice and within 10 days time.
If you are scheduling a couple or family session and can’t seem to get everyone to be at your scheduled session, I encourage you to keep the session for yourself. Your intentions are meaningful and can bring about important change in the situation.
Any behavior reasonably determined to be inappropriate or fabricated will result in termination of your session without refund.
SERVICE PROVIDER
It is the responsibility of the client to choose a provider of their choice for online sessions (i.e. Skype, FaceTime, WebEx, Zoom). Any fees for meeting software beyond Skype or FaceTime are paid by the client. Any WiFi connection charges on the client end are paid and coordinated by the client. If you choose to use Skype, please set up an account at Skype.com and invite “Lorna Acuna” to connect at Lo[email protected]. If we use FaceTime, please connect with Lorna Acuna at [email protected]. If you select Zoom or another platform, please be sure you are familiar with the protocol and recognize our first session may involve connection time on first use. Please provide me with instructions by email in advance at [email protected].
FREE 15-MINUTE CONSULTATION
This session is only conducted by phone. It is a one-time session for new clients to help determine if my services are a good fit for your personal or professional needs.
CLIENT CONFIDENTIALITY
The client understands any confidentiality risks involved in using an online provider. Please review the security encryption statement on the Skype or FaceTime website (or any other provider website that you choose) so that you feel comfortable with the platform and accept any security risks. No audio or video recording of sessions is permitted.
